ADD

Add Forms Application. Created to facilitate accurate gathering of referring physician and insurance company data from imaging centers.  Forms are highly interactive and dynamic as well as visually appealing. To make data collection as smooth as possible, fields default, whole sections appear and disappear and options change in accordance with user input.  Validity checking is user-friendly. Form data is submitted to a database from which ADD and RBO can retrieve via the Administrative Forms Browser, which displays a list of forms submitted and their current status. List can be sorted on any field. Clicking on any item displays the form history (when clicked on date) or the form itself (click anywhere else). Administrative offices may mark forms received as Rejected, Completed or Completed with Problems.  When a form is marked rejected, the submitting party is automatically notified via e-mail with a convenient link allowing him/her to immediately access the form to make corrections and resubmit to ADD.  A Center-Oriented Forms Browser enables Imaging Center personnel to review the status of their requests.  A reporting system allows ADD to monitor the number of times a form is rejected. Maintenance forms (not shown on tabbed interface) allow ADD to control user data used by the ADD Forms Suite and the cc list to control who gets copies of auto-generated e-mails.

Written in HTML, VBScript and JavaScript and utilizing MS Access, SQL, CCDONTS, XML, XSL, CSS and ASP (Active Server Pages) technology and Dynamic HTML.

Status:  Fully functional and in use as of 8/02, but constantly evolving in accordance with user needs and desires.

PURCHASING

Order Forms Suite. Comprises a set of interactive order forms for requesting various supplies, held together with a tabbed interface.  These forms actually convert themselves to e-mail and are automatically sent to the vendor or other party responsible for filling them. (A copy goes to Purchasing)  Business Card form even shows what the finished card would look like with user data in three formats.  Written mostly with DHTML/JScript on the client end with ASP/VBScript/JScript on the server. Uses CCDONTS. Most of the functionality of the forms is on the client end, saving server time.

Universal Order Form (Formerly Wallace Order Form) Presents user with list of supplies he may order.  User simply enters his shipping information (most of which defaults based on pull-down center list) and checks off quantity and shipping preference.  When the form is submitted, a server script creates and sends RTF e-mail to appropriate vendor or vendors.  Used JavaScript, VBScript, ASP and ASPMail component.

Status:  Retired in favor of Order Forms Suite.

Gift Catalog System Used to enable centers and marketing personnel to order approved gifts for referring physician.  User is presented with a two-frame interface, in which the right pane is an illustrated catalog featuring photos, descriptions and prices.  The left pane is an order form which fills in automatically as items are chosen from the catalog.  When the form is submitted the data is forwarded to the Purchasing department as well as saved to a log file.  Also includes reporting system to enable to Purchasing to view a summary of orders in an Excel spreadsheet.  Uses DHTML, JavaScript, VBScript, ASP and ASPMail component. 2000 Version used CCDONTS and XML/XSL.

 

Status:  Intended for use during a limited time during Fall of 1998.  Was completed on time and well received by Purchasing Dept. and users. Reused with modifications for 1999 and redesigned for 2000.

MIS

IS User Form Used to submit to MIS requests for new user accounts. Site data defaults from pull-down list. VISION information section appears automatically if needed.  When submitted, Form converts to e-mail and mails itself to Helpdesk.

Status:  Functional for many months, with few problems.

Requestor/Contact:  Proposed by David Brown and accepted by Help Desk personnel and David Levin, then MIS director. Replaced EXCEL spreadsheet.

 

User Access Database Created on order of David Carroll in 1997 to simulate functionality of VB application being created simultaneously by Santiago Perez.  Point was to automate data gathering for new users of the system.

 

Status:  Retired by David Levin along with the VB version.  Worked in testing, but was never really used.  Replaced with much simpler IS User Form. (see above)

Vision Status Active Desktop Item Meant to display the current status of Vision as a permanent feature of the users’ desktops, in the hopes that such a feature would cut down on inquiries coming into the Helpdesk.  Featured a traffic light metaphor for vision status (red for down, green for up).  A small pane on the side featured a scrolling list of company headlines, in which each item was a hyperlink to further detail.

 

Status: Retired after several months of service.  Desktop Item worked well, but Vision Status data to feed it was not reliable.  Could be reworked into a headline boxed to be updated once or twice a day, without attempting to show Vision Status.

Tutor Vendor  Dispenses Microsoft tutor applications to user, with limit of two per visit to limit bandwidth use. Uses ASP, JScript.

 

Status: Retired by 2000.

Hit Counter Shows number of daily hits to www.usdl.com .  Includes daily total, total since inception, and daily average.  Can be added to active desktop. Uses Perl, ASP and Access.

Press Release Wizard .  Converts Word document to the HTML Format we have been using and sets up all related links on Internet web site.

Status: Active.

HR

 

Safety Manual Application  Interface to official safety manual.  Order was for an interface that would resemble a physical manual, complete with binder rings and tabs along the side to access the various sections.  Also features an interactive index and highlighter functions to highlight key words. Uses DHTML,  JScript, VBScript and MS Index Server. Requires IE 4.0.

 

Status:  Active feature of corporate Intranet.

Computer Based Training/Testing Download Interface to download safety training programs developed by J&H Marsh McClennan.

Status:  Retired.

Safety Incident Reporting Include form for reporting accidents on company premieses, a report “browser” for reviewing such reports, and a report form to summarize incidents.

 

Status:  Functional, but in limbo.  Original contact has left the company, and no further requests have been made for this project. (1999)

Phone List Editor Enables company phone lists to be edited on-line and sent to webmaster for reposting.  Uses Jscipt, DHTML, ASP and ASPMail.

Status: worked great, but not much usage to date. Retired.

BILLING

Medic Reports Interface Facilitates access to MEDIC Reports residing on Web Server in Excel format. Consists of three panes:  a control pane across the top, a report title selector on the left, and a list of available reports by date on the right.  Uses HTML, JScript and ASP.

 

Status:  Retired after several months (over a year?) of faithful service.

Video Survey  Used to gather and log survey data after a video.  Uses ASP, VBScript. JScript.

Status:  One time event.  Worked fine.

Facility List Generator Produces HTML pages for each state in which the company has facilities, based on a database maintained by the webmaster. These pages are posted on the Web Site. Uses MS Access, ASP, DHTML, VBScript and JScript. Also creates pull-down lists for Intranet applications.

Status:  Retired in favor of Facility Data Maintenance (q.v.)

 

Facility Data Maintenance Maintains facility data.Redone to use XML.Produces HTML pages for each state in which the company has facilities, based on a database maintained by the webmaster. These pages are posted on the Web Site. Uses XML, DHTML, VBScript and JScript. Also creates pull-down lists for Intranet applications.

Status:  Used by webmaster whenever site data changes.

 

 

Facility Power Search Enables web site visitors to search the webmaster’s facility database for sites meeting certain modality or geographical criteria.  Uses ASP, VBScript and MS Access.

 

Status: retired in favor of XML/XSL-based views of facility data.

 

Experimental or Under Development:

 

Departmental Calendar - displays a printable calendar in traditional format, with user entered “events.” Mostly experimental at this point, but could be used by departments and centers. Uses XML, VBScript.

 

Date Poster - add dates to calendar. .net, C#, XML. Webmaster initiative with encouragement from Jeff Cozzaglio.

 

Ask Bones - resource finder. XML, Jscript. Webmaster initiative.

 

EOB/Correspondence locator (under development as a .net project. Uses C# and SQL Server 2000)

Job Poster - allows user to submit a job ad for posting to web site.

 

 

Maintenance apps (used within MIS only):

 

Center Data Clearinghouse - maintains information on imaging centers and generates HTML and include files used by Internet and Intranet sites and applications.

 

Web site generator - rebuilds web site from information sources.

 

Index builder - used to build index for web site search tools used on Center web sites. (WSH Script)

 

Seek.asp - home grown XML-based search utility intended primarily for Center Web sites.  To be used with “index builder” (q.v.)

 

Job Ad Editor - used to post, update and remove job ads on web site. Intended for eventual “public” use by Center Administrators or their designates. Allows access to only those ads for which the user is responsible.

 

News Editor - used to post, update and remove news items on center web site. Intended for eventual “public” use by designated Center personnel.

 

Press Release Wizard - posts press release and creates links in web site. Saves time and allows Press Releases to be posted in a more timely fashion.

Announcement Poster - similar to Press Release Wizard, but for announcements to be posted on Intranet.

 

Web Site Document Poster - used to post documents to web site and update the XML files used by the web site to track what’s there and why.

 

Intranet Document Poster - used to post documents to intranet and update the XML files used by the intraet to track what’s there and why.
